Who it's for and when to use it

Use it for anything with a group and a schedule: an SAT bootcamp over four Saturdays, a summer coding camp, a reading club for second graders or a one-off essay writing workshop. Instead of collecting names by text and keeping a list by hand, every sign-up arrives in the same format.

The class and session questions are placeholders, like [Class or workshop 1] and [Session 1 date], which you replace with your real classes and dates. Families pick a class from a dropdown and tick the sessions they'll come to, so you know numbers before the first session.

Parent details are included because most group classes are for children, but the parent name is optional for adult classes. For private lessons, the tutoring registration form is a better fit.

How to use it in 3 steps

  1. Pick the template

Click Use this template, or choose “Group class registration” under New form in Bear Forms. All the questions are already there.

  2. Make it yours

Replace the parts in square brackets, like [Class or workshop 1], with your own details (6 in this template). Change, add or remove any question you like.

  3. Share the link

Publish, then send the link by email or text, put it in your Instagram bio or Google Business Profile, or paste two lines of code to show it on your website.

Tips for better responses

  • Put dates in the options, not the description. Write real dates in the session options, like "Saturday, March 7", so families tick exactly what they'll attend and your headcount per session is accurate.
  • Ask about skill level. The skill level question helps you split a big group, or warn a beginner that a class is advanced. Read it before the first session.
  • Export the list before each session. Download responses as a CSV file and filter by session to get an attendance list with phone numbers.
  • Share it with schools and community groups. Group classes fill through word of mouth. Send the link to parent groups, school newsletters and local community pages, not just your own followers.

Make it your own

  • One class only? Delete the class question and put the class name in the form title, so families only choose sessions.
  • Adult or English learners' groups? For speaking groups, the conversation club sign-up asks about level, topics and how people want to be corrected.
  • Class already full? Switch the link to the tutoring waitlist so latecomers can still leave their details.

Frequently asked questions

What should a group class registration form include?

A group class registration form should include the student's name and grade, contact details for a parent or the student, the class they want, the sessions they'll attend, their skill level and space for questions. This template has all of that in nine questions across two steps.

How do I list my classes and dates on the form?

Replace the bracketed options, like [Class or workshop 1] and [Session 1 date], with your real class names and dates. You can add as many options as you need, or remove ones you don't use.

Can one form handle several classes?

Yes. List every class in the dropdown and every date in the sessions question. Each response shows which class and sessions the student chose, and the CSV export lets you sort sign-ups by class.

Do adults need to fill in the parent question?

No. The parent or guardian name is optional, with a note that it's needed for students under 18, so adults can skip it. Email is still required so you can confirm their place.

Can I edit the questions after I start using it?

Yes. Every question can be changed, removed or reordered at any time, and you can add new ones. When you publish the changes, the same link shows the updated form, so you never have to send families a new link.

Will the form stop spam?

Yes. Bear Forms filters out spam quietly in the background, so real families never have to solve a puzzle or click on pictures of traffic lights before they can send the form.

How much does Bear Forms cost?

Bear Forms costs $4 a month on the Solo plan, or $32 a year. Solo covers up to 30 forms and 1,000 responses a month. Pro is $15 a month or $144 a year for unlimited forms and responses. There's no free plan, but every first payment comes with a 30-day money-back guarantee.
