Lesson Reschedule Request Form Template
This lesson reschedule request form lets students and parents ask to move a lesson without a back-and-forth of texts. They tell you which lesson, why, and when they'd like to meet instead, on one short page.

Who it's for and when to use it
Use it when lesson changes start eating your evenings. A tutor with twenty weekly students might get three or four reschedule requests a week, each arriving in a different app at a different time. One link turns them into the same seven answers, with the original date and a proposed new one.
The reason question is optional and simple: illness, a school event or something else. It isn't there to judge families, but it helps you spot patterns, like a sports season that clashes with Thursday lessons every week.
It works best alongside a clear policy. If you charge for late cancellations, send the tutoring policy agreement when lessons start, and link to it from your reschedule form so families know the rules before they ask.

Tips for better responses
- Make it the only way to reschedule. Tell families that changes go through the form. It feels strict for a week, then it saves you hours, because every request has the same details.
- Mention your notice period. Edit the form's first question or title to remind families of your notice period, like [24 hours'] notice. Most late requests come from people who simply forgot the rule.
- Ask for backup times. The notes box asks for other times that would work. Two or three options means you can confirm in one reply.
- Reply with a confirmation. You get an email alert for each request. Confirm the new time in writing so there's no confusion on the day.
Make it your own
- Several tutors? Add a short answer for the tutor's name, or a dropdown with your tutors, so requests go to the right person.
- Cancellations, not just changes? Add a choice at the top: "Move this lesson" or "Cancel this lesson", and make the new date optional.
- Online lessons? Add a question asking whether the lesson could move online instead, which often saves the slot.
Frequently asked questions
What should a lesson reschedule form ask?
A lesson reschedule form should ask who is asking, the date of the lesson to move, a preferred new date and time, and any other times that work. This template also asks for an optional reason, for seven short questions on one page.
How much notice should students give to reschedule a lesson?
Most tutors ask for 24 hours' notice to reschedule, and some ask for 48. Pick what lets you fill or protect the slot, write it in your policy, and remind families of it on the reschedule form.
Should I charge for late cancellations?
Many tutors charge in full for cancellations with less than 24 hours' notice, because the slot can't be filled. Whatever you decide, put it in writing before lessons start, for example with a policy agreement form.
Can students reschedule from their phone?
Yes. The form is a single page that works on any phone, so a parent can send a request in under a minute.
Do I need any technical skills to use this template?
No. You change questions by clicking on them, and you can drag them into a new order. There's no code to write. If you can fill in a form, you can edit this one, and publishing it is one button.
How do I share the form with families?
Share it as a link or put it on your website. The link works anywhere: email, text messages, WhatsApp, your Instagram bio or your Google Business Profile. To show it on your own site, paste two lines of embed code and the form appears on the page.
Where do responses go?
Every response lands in your Bear Forms dashboard, in a table you can read at a glance, and you get an email alert as soon as it arrives. You can also export all responses to a CSV file that opens in Excel or Google Sheets.
