Who it's for and when to use it

Use it once, when a family starts lessons. The most common tutoring arguments are about late cancellations and missed lessons, and almost all of them start with "I didn't know". A short agreement that parents tick through before the first lesson settles that before it comes up.

Each policy is its own question with the rule underneath, like "Please give at least [24 hours'] notice to cancel or move a lesson", and an "I agree" checkbox that must be ticked. Parents read one rule at a time instead of skimming a long document.

Replace the bracketed parts with your own: your business name, your notice period and how payment works. Then send it after the tutoring registration form, and link to it from your lesson reschedule request.

How to use it in 3 steps

  1. Pick the template

Click Use this template, or choose “Tutoring policy agreement” under New form in Bear Forms. All the questions are already there.

  2. Make it yours

Replace the parts in square brackets, like [Your tutoring business name], with your own details (3 in this template). Change, add or remove any question you like.

  3. Share the link

Publish, then send the link by email or text, put it in your Instagram bio or Google Business Profile, or paste two lines of code to show it on your website.

Tips for better responses

  • Keep each policy to one sentence. Families read short rules. If a policy needs a paragraph, it's probably two policies.
  • Send it before the first lesson. Agreement after a problem feels like a trap. Agreement before lessons start feels like a professional welcome.
  • Point to it when it matters. When a late cancellation happens, reply kindly and quote the policy the family agreed to. It keeps the conversation calm.
  • Review your policies once a year. Notice periods and prices change. Update the form and ask families to agree again at the start of the year.

Make it your own

  • More policies? Add another checkbox question for anything else, like a policy for no-shows or for lessons during holidays.
  • Photos at lessons or events? Send the photo and media consent form alongside it.
  • Adult students? Remove the parent question and ask for the student's own name and email.

Frequently asked questions

What should a tutoring policy agreement include?

A tutoring policy agreement usually covers cancellations and the notice needed, late arrivals, and how and when payment is due. This template includes those three policies, each with an "I agree" checkbox, plus space for questions about them.

What is a fair tutoring cancellation policy?

Many tutors ask for 24 hours' notice and charge in full for later cancellations, while some allow one free late cancellation a term. Choose a rule you'll actually enforce, write it in one sentence and put it in the agreement.

Is a checkbox agreement legally binding?

This template records that a family read and agreed to your policies, but it isn't legal advice and it isn't an e-signature. Rules differ by state, so check with a local professional if you need a formal contract.

Can families ask questions before agreeing?

Yes. The last question invites questions about the policies. Reply before the first lesson so everyone starts on the same page.

How much does Bear Forms cost?

Bear Forms costs $4 a month on the Solo plan, or $32 a year. Solo covers up to 30 forms and 1,000 responses a month. Pro is $15 a month or $144 a year for unlimited forms and responses. There's no free plan, but every first payment comes with a 30-day money-back guarantee.

How hard is it to set up?

It's quick: pick the template, change the parts in brackets, and press Publish. Questions are edited by clicking on them and moved by dragging, so there's nothing technical to learn. Most tutors have a form ready to share in a few minutes.

Can I put this form on my website?

Yes. Copy two lines of embed code into your website and the form appears right on the page. It resizes to fit phones. If you don't have a website, share the form's link instead: by email, by text, in your Instagram bio or on your Google Business Profile.
