Who it's for and when to use it

Use it when a family is clearly happy: after a great report card, a passed exam or a recital. A math tutor whose student just went from a C to an A has the perfect moment to ask, and this form makes it a one-minute job for the parent.

The key question is "What has changed since starting lessons?" It gets you a story, not just "great tutor", and stories are what convince new families. The rating gives you a number to show alongside it.

Permission is built in. Families choose between sharing with their name, first name only, or keeping it private, and a photo is only shared if they said yes. For photos from lessons, use the photo and media consent form.

How to use it in 3 steps

  1. Pick the template

Click Use this template, or choose “Testimonial request” under New form in Bear Forms. All the questions are already there.

  2. Make it yours

Every question is ready to use as it is. Change, add or remove any of them to fit how you work.

  3. Share the link

Publish, then send the link by email or text, put it in your Instagram bio or Google Business Profile, or paste two lines of code to show it on your website.

Tips for better responses

  • Ask at a high point. Send the link the day the good news arrives. Waiting a month means the excitement, and the detail, is gone.
  • Make it personal. Send the link in a short personal message, like "Would you mind sharing a few words about Sam's progress?" A personal ask gets far more replies than a mass email.
  • Respect the sharing choice. Check the sharing answer before you post anything, and use first names only when that's what they chose.
  • Thank them either way. Some families will choose to keep their review private. Their feedback is still useful, so thank them for it.

Make it your own

  • Want broader feedback? The parent satisfaction survey asks about satisfaction, communication and whether they'd recommend you.
  • Adult learners? Change "I am a" to include "Adult learner", and replace the photo question if you don't need it.
  • Video testimonials? Change the photo upload to accept video files instead.

Frequently asked questions

How do I ask parents for a tutoring testimonial?

Ask personally, right after a success like a better grade or a passed exam, and send a short form instead of asking for an email. This template asks for a rating, what has changed and a favorite moment, so parents know what to write.

Do I need permission to use a testimonial?

Yes, you should get clear permission before you publish someone's words. This form asks whether you may share the testimonial with their name, with their first name only, or not at all, so you have their choice in writing.

What makes a good tutoring testimonial?

A good tutoring testimonial describes a specific change, like a grade going up or a child who stopped dreading math. The question "What has changed since starting lessons?" invites exactly that kind of story.

Can parents add a photo?

Yes. The form has an optional photo upload, and its help text says the photo is only shared if the family agreed to sharing. You can remove the question if you don't use photos.

Where do responses go?

Every response lands in your Bear Forms dashboard, in a table you can read at a glance, and you get an email alert as soon as it arrives. You can also export all responses to a CSV file that opens in Excel or Google Sheets.

Can I change the form later?

Yes, at any time. Edit, add or remove questions, then publish again. The link stays the same, so the form on your website or in your bio updates for everyone.

Does the form work on phones?

Yes. Most parents open forms on their phones, so the form fits any screen size. Multi-step forms show a progress bar, and file uploads work from a phone's camera roll.
