Who it's for and when to use it

Use it twice a year, or at the end of each term, to measure how families feel. It's especially useful for tutoring agencies: the tutor's name is a question, so you can compare tutors fairly and spot who needs support.

Solo tutors can use it too. The recommend score, from 1 to 10, is a simple number to track over time: if it drops, something changed, and the open questions usually tell you what.

The parent's name is optional, so families can answer anonymously. For a quick monthly pulse instead of a survey, use the parent progress check-in.

How to use it in 3 steps

  1. Pick the template

Click Use this template, or choose “Parent satisfaction survey” under New form in Bear Forms. All the questions are already there.

  2. Make it yours

Every question is ready to use as it is. Change, add or remove any of them to fit how you work.

  3. Share the link

Publish, then send the link by email or text, put it in your Instagram bio or Google Business Profile, or paste two lines of code to show it on your website.

Tips for better responses

  • Keep names optional. Parents are more honest about a tutor when they don't have to sign their name. Leave the name question optional.
  • Track the recommend score each term. Export responses to a CSV file and average the recommend score by term and by tutor. The trend matters more than any single number.
  • Share good feedback with tutors. Pass on what families say a tutor does well. It's free motivation, and it makes the survey feel positive rather than like an inspection.
  • Close the loop. Tell families one thing you changed because of the survey. Response rates go up the next time.

Make it your own

  • Solo tutor? Delete the tutor's name question and change "we" to "I" where it fits.
  • More detail on progress? Add a star rating for confidence, or a question about homework load.
  • Want reviews? Families with high scores are good candidates for the testimonial request form.

Frequently asked questions

What should a parent satisfaction survey for tutoring ask?

It should ask about overall satisfaction, punctuality, communication, the progress parents have seen and how likely they are to recommend you, plus what you do well and what to improve. This template asks those nine questions in two short steps.

Why is the recommend question 1 to 10?

A 1 to 10 scale gives a more detailed answer than five stars, so small changes between terms show up. Track the average each term; a falling score is an early warning.

Can parents answer anonymously?

Yes. The parent name is optional, so families can leave it blank. Only the tutor's name and the ratings are required.

How often should tutoring agencies survey parents?

Once or twice a year is enough for most agencies, usually at the end of a term. More often than that and parents stop answering; use a short monthly check-in for anything in between.

Where do responses go?

Every response lands in your Bear Forms dashboard, in a table you can read at a glance, and you get an email alert as soon as it arrives. You can also export all responses to a CSV file that opens in Excel or Google Sheets.

Can I change the form later?

Yes, at any time. Edit, add or remove questions, then publish again. The link stays the same, so the form on your website or in your bio updates for everyone.

Does the form work on phones?

Yes. Most parents open forms on their phones, so the form fits any screen size. Multi-step forms show a progress bar, and file uploads work from a phone's camera roll.
