Who it's for and when to use it
Use it in the last two weeks of a term or school year. It does two jobs at once: it gives you honest feedback on the term, and it tells you who's coming back, so you can plan next term's schedule before it starts.
A tutor with fifteen families can send one link in early June and know by the end of the month which slots are free for September, which families want different times, and which ones need a conversation.
Families who rate the term highly and mention a big improvement are great people to ask for a review next, with the testimonial request form. For a lighter monthly check, use the parent progress check-in.

Tips for better responses
- Send it before the last lesson. Responses arrive while you can still talk about them in person. After the last lesson, replies drop off.
- Follow up on "Not sure yet". Families who aren't sure are the ones you can still keep. A short call about next term's goals often decides it.
- Plan next term from the times question. Export responses to a CSV file and sort by preferred times to build next term's schedule.
- Act on one suggestion publicly. Pick one improvement families asked for and tell everyone you've made it. It shows the form matters.
Make it your own
- Several tutors? Add a tutor name question, or use the parent satisfaction survey, which rates the tutor on punctuality and communication.
- Group classes? Add a dropdown for the class so you can compare feedback between classes.
- Students' view too? Send learners the monthly learner self-review at the same time.
Frequently asked questions
What should an end-of-term evaluation ask?
An end-of-term evaluation should ask how much progress the student made, how communication went, the biggest improvement, what could be better, and whether the family is continuing next term. This template asks all of that in nine questions over two steps.
When should I send an end-of-term evaluation?
Send it one or two weeks before the last lesson of the term. Families still remember the term clearly, and you get the answers in time to plan next term's schedule.
Can I use it to plan next term?
Yes. The second step asks whether the family is continuing and which times suit them next term, so the responses double as early sign-ups.
Should the evaluation be anonymous?
This one isn't, because you need to know who is continuing. The parent name is optional, but the student name is required. For anonymous feedback, remove the names and the continuing question.
Do I need any technical skills to use this template?
No. You change questions by clicking on them, and you can drag them into a new order. There's no code to write. If you can fill in a form, you can edit this one, and publishing it is one button.
How do I share the form with families?
Share it as a link or put it on your website. The link works anywhere: email, text messages, WhatsApp, your Instagram bio or your Google Business Profile. To show it on your own site, paste two lines of embed code and the form appears on the page.
Where do responses go?
Every response lands in your Bear Forms dashboard, in a table you can read at a glance, and you get an email alert as soon as it arrives. You can also export all responses to a CSV file that opens in Excel or Google Sheets.
